Instructions for Completing Your Pre-entrance Health Requirements For:
Undergraduate, Transfer and AAP Post-baccalaureate Students

             

       
The deadline for compliance with the University's pre-entrance health requirements is: 

  

            Fall Semester:  July 15

          Spring Semester:  January 15

       Post-bacc students:  June 1

                                      
                     

YOU WILL BE UNABLE TO COMPLETE THE ORIENTATION PROCESS OR ADD/DROP CLASSES UNTIL YOU COMPLETE STEPS A, B AND C.

 

A  All entering students are required to provide proof of immunity to certain communicable diseases by submitting a Pre-Entrance Immunization form completed & signed by your health care provider.  You must also complete and sign the Consent to Medical Treatment/Insurance Information form prior to registration.  

 

Information regarding meningitis must be reviewed before signing the waiver statement on the Immunization form. You can consult your primary health care provider for information on meningitis vaccine.  In addition, below are links to information on the meningitis provided by the CDC (Centers for Disease Control and Prevention) and the National Meningitis Foundation.

AND

B Enter your immunization information and complete a short medical history online at the JHU SHWC Web Portal.  The information you enter will download directly into your personal JHU electronic health record.  You will need the following information to access the web portal and enter your information on-line:

1)  JHED Login ID (e.g., lzeigle2):  Please check your acceptance letter or obtain this information from your program co-ordinator. 

2)  Hopkins ID (comprised of six letters and/or numbers):  For instructions on how to obtain your Hopkins ID please click on this link Registrar's website, "Welcome to ISIS".

3)  Email address:  The password needed to enter the web portal will be sent to the e-mail address you provide when completing your web portal registration.  We prefer you use your JHU e-mail, but if you have not set it up, you can use your personal e-mail address.

4)  Immunization Form:  If you have a record of your immunizations, you can start the on-line process at any time; otherwise, we suggest you wait until your health care provider has reviewed and signed the Pre-entrance Immunization form.  Please keep a copy of the immunization form for your records.

Please be sure you have met all of the immunization requirements to avoid any interruption to your Orientation schedule when you arrive at JHU. If you arrive on campus without a completed Pre-entrance Health form, the Student Health & Wellness Center will complete the form for you so that you will be allowed to complete your Orientation. There is a fee of $85.00, plus the cost of any necessary vaccines, to complete your health form.

  

Instructions for Obtaining Your Hopkins ID Number (ISIS ID) for Access to the SHWC Web Portal
 

Step 2: How Do I Find My Hopkins ID?

Your Hopkins ID is a six character alphanumeric identifier that uniquely identifies a member of the Johns Hopkins community. It is intended to replace social security numbers, thus reducing the risk of identity theft. The Hopkins ID is not used for authentication or access to any systems or services, and, like names, it is neither private nor confidential. It will be used solely by institutional officials in carrying out official business activities.

 

                      1.  Go to http://www.jhu.edu/registrar

                 2.  Select GO RIGHT TO LOG-IN and click on "Sign in"  using your JHED ID &

                           password.  (If you  have not obtained your JHED ID and password, see

                           instructions immediately preceeding this page.)  

                      3.  Point to Personal Info at the top of the screen and select Summary. Your

                           Hopkins ID is under Biographic Information in the middle column. If you
