Software Request Form
- Check to make sure the application you are requesting isn't already installed.
- Review the Guidelines and End User Agreement below.
- Fill out and submit the request form below.
- Send the following items to Jason Postelthwait - 165 Krieger Hall, 410-516-2923 or jwp@jhu.edu:
- Application Media (CD, DVD or url to download)
- Copy of License Agreement or proof of ownership
- Special installation requirement instructions
Application Installation Request Guidelines
- Only Hopkins faculty or staff can request that software be installed in the computer labs.
- A separate request must be made for each application.
- All software installed on computer lab machines must be site-licensed or licensed for a specific number of machines in the desired computer lab or classrooms.
- Software to be installed must not interfere with the existing systems.
- The requesting department or instructor must be available for consultation with ITF staff on configuration issues.
End User Agreement
- I am responsible for testing the software installation in the computer lab or classrooms after it has been installed to make sure it functions properly.
- I understand that the turnaround time for software installations may take up to 10 business days in the public computing labs and 2 business days in the computer classrooms.
- I am responsible for renewing software installations each academic term; otherwise the software may be removed.
- I verify that I am licensed to use the number of copies indicated on this form and I understand that this application will be accessible from the computers in the lab(s) in which the software is installed.
- I understand that ITF is not responsible for any supporting documentation and other materials for the application you have requested to be installed.
